The inquest touching upon the sad death of Makenna-Rose Thackray concluded today with the Jury returning a narrative conclusion.
The Jury concluded that there were missed opportunities to save the life of 4-year-old Mekenna-Rose Thackray.
Makenna-Rose was diagnosed with Scarlett Fever on the 9th December 2022 but was only prescribed a 7 day course of antibiotics rather than the recommended 10 day course. Makenna-Rose continued to be unwell and deteriorated further on the 18 December when she vomited blood and was rushed by ambulance to Pinderfields Hospital.
Upon arrival at hospital Makenna-Rose was triaged and scored a 7 on the Paediatric Advanced Warning Score(PAWS) which required her to be assessed by a doctor within 20 minutes or a consultant within 30 minutes and to have hourly clinical observations. Makenna-Rose was not assessed, nor did she have hourly observations. The hospitals standard operating procedure also required a sepsis screening tool to be completed, this was not done either.
Makenna-Rose and her parents waited for over 4 hours to be seen before going home with the agreement that they would visit their GP the following day. They indeed visited their GP the following day who diagnosed influenza and provided reassurance. The family of course informed the GP of Makenna-Rose’s deterioration and that she had been rushed to hospital in an ambulance the previous night, something which he denied.
On the 20th December Makenna-Rose vomited a large amount of blood, collapsed, and suffered a cardiac arrest. Her parents tried in vain to provide CPR, but Makenna-Rose was sadly pronounced deceased at 21:10 that night.
Makenna-Rose was a larger than life character, those who met her would never forget her. She was a real daddy’s girl and loved her pet rabbit and dog, Floss.
